Nowra Community Health Centre - Cardiac Services

The Nowra Community Health Centre provides cardiac services for patients living from Gerringong to Milton. The Centre provides the following services:

Cardiac Rehabilitation

The Shoalhaven Cardiac Rehabilitation Service is a community based secondary prevention program for people with heart disease and their families.  Patients with cognitive impairment are accepted if carers require education and support to manage their condition.

A plan of overall Cardiac Rehabilitation care will be formulated and can comprise of a mix of home, group, clinic or phone based interventions by the Cardiac Rehabilitation nursing and exercise physiology staff.  Referral to other agencies: diabetes education, GP, cardiologist or mental health professional is initiated as needed.

We are supported by allied health staff from SDMH to facilitate the group education and discussion sessions.  Education packages can be mailed to patients as per their management plan.  The patients support persons or carers are automatically involved in the patients cardiac rehabilitation care

Heart Failure Service

The Heart Failure Service is a community-based program that provides support for patients with heart failure, their families and carers. Our team comprises of specialist heart failure nurses and exercise physiology staff. We offer referral to other chronic disease support programs to support our patients as needed. This service is provided free of charge. 

The aim of the program is to improve the quality of life for patients and their families or carers and to reduce the incidence of avoidable hospital admissions. Our team promotes best practice as per the National Heart Foundation of Australia and the Cardiac Society of Australia and New Zealand; Guidelines for the prevention, detection and management of chronic heart failure in Australia (2011).

Services offered:

• Health management education; group education and exercise programs; prescription of individualised therapeutic exercise regimes and self-management strategies.
• Outpatient clinics at NCHC and Milton Ulladulla Hospital
• Inpatient assessment and consultation
• Post discharge telephone follow-up and face to face assessment.
