You are eligible to access this service if you have a renal condition and meet the referral criteria.

How to access this service (referrals or no referral necessary)

You must have a referral from a health professional such as a Nephrologist, General Practitioner, nurse, dietitian or other health professional.  

What to bring

You don’t need to bring anything to this appointment. You may like to bring any recent blood test results, a record of what you eat or a medication list, although these are not essential.
