Osteoporosis Refracture Prevention Service

Overview

A Multidisciplinary Refracture Prevention Service designed to identify your risk of further broken bones, start treatments for your bone health, education about your bone health, lifestyle factors and development of a personalised management plan. The service is for the Illawarra region Helensburgh to Gerroa.

What we do

We offer bone mineral density testing on our bone mineral density – Dual energy x-ray absorptiometry machine. Medical Assessment and review, Multidisciplinary assessments, and follow up at 3, 6 and 12 months post appointments.

When to access this service

Patients discharged from ISLHD emergency departments, acute or subacute wards, requiring investigation of their bone health.

What to expect

You will have a bone mineral density scan (BMD), see a doctor and a fracture liaison coordinator in 2 separate appointments. You will have follow up with the coordinator at 3, 6 and 12 months either phone call or face to face appointment as required. You will have repeat BMD at either 2 or 5years depending on your results.

You will also be referred to other outpatient departments as appropriate.

Who should access this service

Patients that are either over 50years of age, or 40 years of age and identify as Aboriginal or Torres Strait Islander and  have experienced a minimal trauma fracture, which is a slip trip or fall from standing height resulting in a fracture.

How to access this service (referrals or no referral necessary)

You will need a written referral from your doctor to use this service. All health professionals can refer with relevant doctor completed referral form.

What to bring

  • Veteran Affairs Card, NDIS Plan, Medicare Card
  • List of current medications
  • Good walking shoes and usual walking aid.
  • For BMD scan Loose comfortable clothing is suggested, avoiding zippers, buckles and metal buttons if possible.

Port Kembla Hospital is located on Cowper Street. Pedestrians can access the hospital from the main entrance on Cowper Street. There is a bus stop located close to the hospital with the following services available – Premier Illawarra 52, 53 and 54.

There is limited parking and disabled parking on site and surrounding streets.

Limited car parking spaces are available at the front entrance from Cowper Street or via Fairfax Road to the southern car park and the lower western car park.
