The dietitian will complete a nutrition assessment and provide nutrition education for your condition

A dietitian will ask you some questions about your history, take some measurements, assess your diet and provide appropriate dietary education and counselling for your condition.

Who should access this service

You are eligible to access this service if you are ≤16 years old and meet the referral criteria.

How to access this service (referrals or no referral necessary)

You must have a referral from a Paediatrician or have seen a dietitian while in hospital.

What to bring

You may like to bring a record of what you eat in a normal day, though this is not essential. Your dietitian will ask this on the day.
