Overview
What the service is
The Occupational Therapy team's main focus is to develop and maintain a person’s skills to carry out their everyday occupations such as self-care tasks and leisure activities. It is our role to ensure that the patients have everything they need to be able to carry out their everyday tasks safely after they are discharged home from hospital.
What we do
- Allied Health – physiotherapy and occupational therapy only;
- Individual consults to patients either in the clinic or in their homes;
- Assessment and facilitation to achieve person-centred goals;
- Provide allied health outpatient service to participants of AAA group.
What to expect
Comprehensive assessment of the older person, encompassing physical, cognitive and functional ability, with carer advice and support.
How to access this service
Referral from GP or on discharge from hospital for individual Geriatrician consultation.
Referral from Geriatrician to attend AAA group program.
