Making an in-memoriam donation
Making an in-memoriam donation is a powerful way to honour the life of a friend or loved one.
You may visit our online payment portal (Be sure to select Illawarra Shoalhaven Local Health District as your Health Organisation in the drop-down menu.)
Or simply download this form and follow the instructions that work best for you.
If you have questions, please write our District Fundraiser at ISLHD-Fundraising@health.nsw.gov.au.
Making a donation instead of flowers
You may wish to ask friends and family to give a donation instead of flowers at the funeral or memorial service of a loved one.
We suggest you include a request in your loved one's death notice.
The request may look like:
“Donations instead of flowers to Illawarra Shoalhaven Local Health District”.
You may also wish to specify a particular hospital or department e.g. Wollongong Hospital Renal Unit after selecting Illawarra Shoalhaven Local Health District under 'Health Organisation' in the form's drop-down menu.
These collections can be made as a donation following the directions on this webpage.
Organising a collection at a funeral service
