Making an in-memoriam donation is a powerful way to honour the life of a friend or loved one.
Select the 'In Memory' option on our online payment form or download this form and send a cheque or money order to us via:

Please do not send cash or credit card details by mail.

Making a donation instead of flowers
You may wish to ask friends and family to give a donation instead of flowers at the funeral or memorial service of a loved one.
We suggest you include a request in your loved one's death notice.
The request may look like:
“Donations instead of flowers to Illawarra Shoalhaven Local Health District”.
You may also wish to specify a particular hospital or department e.g. Wollongong Hospital Renal Unit.
Donations can be made on our online payment form.
Organising a collection at a funeral service
You may wish to ask friends and family to make a donation at the funeral or memorial service of a loved one through a collection.
Funds raised through a collection can be taken over the phone or you may have those wishing to make an in-memoriam donation print and mail gifts with this In Memory Donation Form. If you would like help choosing an area to send your gift, please call (02) 4221 6899 and ask for the District Fundraiser, or email ISLHD-Fundraising@health.nsw.gov.au.
