Making an in-memoriam donation
Making an in-memoriam donation is a powerful way to honour the life of a friend or loved one.
Visit our Ways to donate section for more information on how to make your gift, or simply download this form and follow the instructions that work best for you.
If you have any questions, please contact the Fundraising and Donations Coordinator on (02) 4221 6894 or ISLHD-Fundraising@health.nsw.gov.au.
Making a donation instead of flowers
You may wish to ask friends and family to give a donation instead of flowers at the funeral or memorial service of a loved one.
We suggest you include a request in your loved one's death notice.
The request may look like:
“Donations instead of flowers to Illawarra Shoalhaven Local Health District”.
You may also wish to specify a particular hospital or department, for example
“Donations instead of flowers to Wollongong Hospital Renal Unit”.
You can also provide this form to family and friends so they can make a donation to us in their own time.
Organising a collection at a funeral service
Funds raised through a collection in memory of your loved one at a funeral or memorial service can also be donated to ISLHD.
Please see our Ways to donate page for more information.
