Sustaining NSW Families Team

Overview

Who we are    

The Sustaining NSW Families (SNF) home-visiting program helps families who are pregnant and parents of newborns who: 

  • need help with the health and development of their baby
  • struggling with mental health and wellbeing

Our team includes:

  • Child and Family Health nurses
  • social workers
  • allied health professionals.

The program is free to families who live in the Illawarra Shoalhaven Local Health District.
 

What we do    

Our team works with you and your family to: 

  • use your strengths and knowledge to meet your baby’s needs
  • support the health and development of your baby
  • support you in the parenting journey.

As part of the program, you and your family will have a dedicated Child and Family Health nurse who will come to your home to support you until your baby’s second birthday.

The program will not replace your midwife or medical officer for pregnancy care.

 

How to get help

  • The program is voluntary and provided free by NSW Health.
  • To take part in the program, you will need a referral.
  • Referrals are usually made by the maternity or psychology service you are using.
  • The referral is usually made during pregnancy or shortly after giving birth.
