Please bring to all your diabetes appointments:
- Any paperwork relating to your appointment eg: GP letter, pathology results and blood glucose levels in a record book.
- A list of all the medications (tablets and injections) you take.
- Your Medicare and/or Department of Veteran Affairs (DVA) card.
